Privacy Policy

Last updated: September 2, 2026

Jive Health, a brand operated by 10ME LLC (“Jive Health,” “Company,” “we,” “us,” or “our”) respects your privacy. This Privacy Policy explains what information we collect through our websites, apps, and services (collectively, the “Services”), how we use and share it, and the choices you have. By using the Services, you agree to the practices described here.

This page also includes:

  • our Consumer Health Data section, which supplements this Policy for Washington and Nevada residents under Washington’s My Health My Data Act and Nevada’s consumer health data law; and
  • the Notice of Privacy Practices of MD Integrations, Inc., the medical group that provides clinical services through the platform.

Information We Collect

Information you provide

  • Account and contact information — name, email address, phone number, shipping address, date of birth, and login credentials.
  • Health and intake information — responses to health questionnaires, medical history, medications, photos you submit, and communications with your care team.
  • Payment information — collected and processed by our payment processors; we do not store full card numbers.
  • Customer support content — messages, reviews, testimonials, and survey responses you choose to share.

Information collected automatically

  • Device and usage data — IP address, browser and device type, operating system, pages viewed, referring URLs, and interactions with the Services.
  • Cookies and similar technologies — used for essential site functions, analytics, personalization, and advertising. You can control cookies through your browser settings; disabling some may limit functionality.
  • Approximate location — inferred from IP address to confirm service availability in your state.

Prescription and treatment information

When you receive care through the Services, the Medical Group (defined below), its Providers, and partner pharmacies generate clinical and prescription records about you.

A Note About HIPAA and Protected Information

Clinical services available through the Services are provided by MD Integrations, Inc. and its affiliated professional entities (mdintegrations.com) (collectively, the “Medical Group”) and the licensed clinicians who practice with them (the “Providers”).

10ME LLC is generally not a “covered entity” under the Health Insurance Portability and Accountability Act of 1996 and its implementing regulations (“HIPAA”). However, HIPAA does not apply (or not apply) to information simply because it relates to health — it depends on who holds it and how it is used:

  • Health information you submit through the platform that is held by 10ME LLC is protected as described in this Policy and applicable state law.
  • Information held by the Medical Group, its Providers, and partner pharmacies may constitute “protected health information” (“PHI”) governed by HIPAA and by those entities’ own privacy notices.
  • In providing administrative, operational, and technology services to the Medical Group and partner pharmacies, 10ME LLC may act as a “business associate” under HIPAA. Solely in that role, we are subject to certain HIPAA requirements with respect to PHI we handle on their behalf.
  • Any medical or health information that is subject to specific protections under applicable state laws (together with PHI, “Protected Information”) will be used and disclosed only in accordance with those laws. Information that does not constitute Protected Information may be used and disclosed as described in this Policy. Protected Information does not include information that has been de-identified in accordance with HIPAA.

The Medical Group and its Providers have adopted a Notice of Privacy Practices (below) describing how they use and disclose Protected Information. By accessing or using the Services, you acknowledge receipt of that Notice.

By using the Services, you agree that even where HIPAA applies, information you submit to us that is not intended and used solely for the provision of diagnosis and treatment by the Medical Group and Providers, or for prescription fulfillment by the partner pharmacies, is not Protected Information and is subject to this Policy and applicable state law.

How We Use Information

  • to provide, operate, and improve the Services;
  • to create and manage your account and fulfill your orders;
  • to facilitate care between you and the Medical Group and its Providers;
  • to process payments and prevent fraud;
  • to communicate with you about your care, account, and — with your consent where required — marketing;
  • to perform analytics, research, and product development;
  • to personalize content and measure advertising effectiveness; and
  • to comply with legal obligations and enforce our terms.

We may de-identify information so it can no longer reasonably identify you and use or share it for any lawful purpose; we commit not to attempt to re-identify it.

How We Share Information

  • With the Medical Group, its Providers, and partner pharmacies to deliver your care and fulfill prescriptions.
  • With service providers that support our operations (hosting, analytics, communications, payments, shipping) under contractual confidentiality obligations, including in our capacity as a business associate where applicable.
  • With our affiliates and subsidiaries.
  • For legal reasons — to comply with law, respond to lawful requests, defend our rights, or protect the safety of any person.
  • In a corporate transaction such as a merger, financing, or sale of assets.
  • With your consent or at your direction.

We do not sell your personal information for money. Where our use of advertising and analytics tools constitutes a “sale” or “sharing” under certain state laws, you can opt out as described below.

SMS and Email Communications

With your consent, we send text messages and emails about your care, orders, and offers. Consent to receive marketing texts is never a condition of purchase. Message and data rates may apply; message frequency varies. Reply STOP to any text to opt out of marketing texts; use the unsubscribe link to leave marketing emails. Transactional messages related to active care may continue. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes; text-messaging opt-in data and consent will not be shared with any third parties. Text messages and emails are not encrypted; if you choose to send or receive health information by text or email, you do so at your own risk.

How We Protect Information

We use administrative, technical, and physical safeguards appropriate to the sensitivity of the information, including encryption in transit and access controls. No system is perfectly secure; please protect your credentials and notify us of any suspected unauthorized access.

Retention

We keep personal information for as long as needed to provide the Services, meet legal and clinical record-keeping requirements, resolve disputes, and enforce agreements, after which it is deleted or de-identified.

Your Choices and Rights

  • You may access and update account information through your account or by contacting us.
  • You may request access to, correction of, or deletion of your personal information, subject to legal record-keeping exceptions. For clinical records held by the Medical Group, you may also exercise the rights described in the Notice of Privacy Practices below.
  • You may opt out of marketing communications at any time.
  • Depending on your state of residence, you may have additional rights described below.

To exercise any right, email privacy@jivehealth.co. We verify identity before acting on requests and will respond within the time required by law. Authorized agents may submit requests where state law allows. If we decline a request, you may appeal by replying to our decision.

NOTICE OF PRIVACY PRACTICES FOR MD INTEGRATIONS, INC.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

This Notice of Privacy Practices (the “Notice”) tells you about the ways we may use and disclose your protected health information (“PHI”) and your rights and our obligations regarding the use and disclosure of your PHI. “We” or “MDI” refers to MD Integrations, Inc. dba MD Integrations.

Uses and disclosures of your protected health information that do not require an authorization

Treatment.

For example, doctors, nurses, and other staff members involved in your care will use your PHI to coordinate your care or to plan a course of treatment for you.

Payment.

For example, we may disclose information regarding your medical procedures and treatment to your insurance company to arrange payment for the services provided to you.

Health Care Operations.

For example, we may disclose your PHI for billing or interpreter support. We may use your PHI to conduct an evaluation of the treatment and services provided or to review staff performance.

Communicating with You.

We will use your PHI to communicate with you about a number of important topics, including information about your care, treatment options and other health-related services. We may also contact you at the email, phone number or address that you provide, including via text messages, for these communications. If your contact information changes, it is important that you let us know. We will only provide you a link via text or e-mail which will require you to verify yourself prior to opening the message within the MDI messaging portal.

Business Associates.

At times, we need to disclose your PHI to persons or organizations outside MDI who assist us with our payment/billing and health care operations. We require these business associates and their subcontractors to appropriately safeguard your PHI.

Other Uses and Disclosures.

We may be permitted or required by law to make certain other uses and disclosures of your PHI without your authorization. Subject to conditions specified by law, we may release your PHI:

  • for any purpose required by law;
  • for public health activities, including required reporting of disease, injury, birth and death, for required public health investigations, and to report adverse events or enable product recalls;

  • to coroners, medical examiners and funeral directors;
  • if necessary to arrange organ or tissue donation or transplant;
  • for national security, intelligence, or protective services activities; and
  • for purposes related to your workers’ compensation benefits.

Uses and disclosures of your protected health information based on a signed authorization

Except as outlined above, we will not use or disclose your PHI for any other purpose unless you have signed a form authorizing the use or disclosure. You may revoke an authorization in writing, except to the extent we have already relied upon it.

In some situations, a signed authorization form is required for uses and disclosures of your PHI, including:

  • most uses and disclosures of psychotherapy notes;
  • uses and disclosures for marketing purposes;
  • disclosures that constitute the sale of PHI; and
  • uses and disclosures for certain research protocols as required by privacy law. The confidentiality of substance use disorder and mental health treatment records as well as HIV-related information maintained by us is specifically protected by state and/or federal law and regulations. Generally, we may not disclose such information unless you consent in writing, the disclosure is allowed by a court order, or in other limited, regulated circumstances.

Your rights

Access to Your PHI.

You can access and inspect paper or electronic copies of certain PHI that we maintain about you. You may readily access much of your health information without charge using the following link: https://privacy.mdintegrations.com. In line with set fees under federal and state law, we may charge you for a copy of your medical records.

Amendments to Your PHI.

You can request amendments, or changes, to certain PHI that we maintain about you that you think may be incorrect or incomplete. All requests for changes must be in writing, signed by you or your representative, and state the reasons for the request. If we decide to make an amendment, we may also notify others who have copies of the information about the change. Note that even if we accept your request, we may not delete any information already documented in your medical record. You can make such requests by contacting support@mdintegrations.com or directly inside your MDI patient portal under the “privacy center” section.

Accounting for Disclosures of Your PHI.

In accordance with applicable law, you can ask for an accounting of certain disclosures made by us of your PHI. This request must be in writing and signed by you or your representative. An accounting will include disclosures made in the six years prior to the date of a request.
