Telehealth Consent

Last updated: September 1, 2026

BY USING THE SERVICES, CREATING AN ACCOUNT, OR OTHERWISE AFFIRMATIVELY ACCEPTING THIS CONSENT, YOU ACKNOWLEDGE THAT YOU HAVE READ, ACCEPTED, AND AGREE TO BE BOUND BY THIS CONSENT. IF YOU DO NOT AGREE, DO NOT CREATE AN ACCOUNT OR USE THE SERVICES.

IF YOU ARE EXPERIENCING A LIFE-THREATENING SITUATION, CALL 911. IF YOU ARE IN A MENTAL HEALTH CRISIS OR CONTEMPLATING SUICIDE, CALL OR TEXT THE 988 SUICIDE & CRISIS LIFELINE AT 988.

This Telehealth Consent (this “Consent”) applies to clinical services you receive through the Jive Health platform from MD Integrations, Inc. and its affiliated professional entities (collectively, the “Medical Group”) and its licensed clinicians (the “Providers”). The platform itself is operated by Jive Health, a brand operated by 10ME LLC (“Jive Health,” “Company,” “we,” “us,” or “our”), which does not practice medicine.

Telehealth means the remote delivery of healthcare using technology — the delivery of health care services using electronic communications between a health care provider and a patient who are not in the same physical location. Telehealth may be used for diagnosis, treatment, follow-up, and patient education, and may include: the electronic transmission of medical records, photos, personal health information, and other data between you and your Provider; interactions via secure forms, asynchronous messaging, and live audio or video visits; and the use of data from medical devices and sound or video files. Alternative methods of care, such as in-person services, may be available to you, and you may choose an alternative at any time — discuss alternatives with your Provider.

The systems used in the services incorporate network and software security protocols designed to protect the privacy, security, and integrity of your health information in transit and at rest.

Possible benefits of telehealth

  • Telehealth can make accessing medical care easier, more efficient, and less expensive.
  • You can obtain medical care and treatment at times that are convenient for you, without traveling to an office or sitting in a waiting room.

Possible limitations and risks of telehealth

  • Information transmitted to your Provider may not be sufficient to allow appropriate medical decision-making, and your Provider may not be able to treat your condition via telehealth. You may be required to seek alternative or in-person care.
  • A remote clinician cannot perform a hands-on physical examination or assess vital signs in person, which may in some cases prevent diagnosis or treatment or the identification of a need for urgent medical care. Telehealth is not a replacement for an ongoing relationship with an in-person primary care physician.
  • An accurate evaluation depends on the accuracy of what you share. You must provide a complete and truthful medical history, including every diagnosis, medication, supplement, and prior treatment. Omissions can lead to misdiagnosis, inappropriate treatment, or harm.
  • Technology can fail or contain errors. Connection problems, bugs, or service interruptions may delay evaluation or treatment, limit functionality, or cause records, transmissions, data, or content to be corrupted or lost.
  • Despite safeguards, no electronic system is immune from the risk of unauthorized access to or disclosure of your information.
  • Due to the nature of the services and regulatory requirements in certain jurisdictions, your treatment options — especially pertaining to certain prescriptions — may be limited. State law governs telehealth and remote prescribing, and the services available to you may vary based on where you are located.
  • Telehealth is never appropriate for emergencies. If you are experiencing a medical emergency or a severe allergic or adverse reaction, call 911 or go to the nearest emergency room immediately.

Your Consent to Telehealth

By accepting this Consent, you acknowledge your understanding and agreement to each of the following.

Provider qualifications and service limitations

  • You have read this Consent carefully and understand the risks and benefits of telehealth in your medical care, and you give your informed consent to receive medical care by telehealth from Providers affiliated with the Medical Group through the Jive Health platform.
  • Your care is delivered remotely by telehealth. The clinician treating you may be a nurse practitioner or physician assistant rather than a physician, and you consent to treatment by these providers.
  • Telehealth technology evolves; the tools used in your care may differ from those described here.
  • No outcome is guaranteed — including outcomes of any laboratory testing — and your condition may fail to improve or may worsen. Some conditions cannot be safely treated by telehealth.
  • The services enable coordination and communication with a Provider and do not replace your relationship with any existing health care provider.

Your responsibilities

  • You have considered the benefits, risks, and alternatives to telehealth described above.
  • You have a duty to answer questions about your health and medical history honestly and accurately, and to keep your health care providers up to date on changes in your health, symptoms, treatments, or medications. You understand that withholding or providing inaccurate information in order to obtain treatment may result in serious harm, including in some cases death.
  • Visits are not recorded by the Medical Group.
  • Your Provider will evaluate your condition and decide, in their sole professional judgment, whether telehealth treatment is appropriate for you, and you accept that determination. Your Provider may conclude your condition requires in-person care.
  • You agree to provide accurate, current, and complete medical information — including records from other providers on request — and current emergency contact information.
  • You agree to monitor and respond to messages, reports, and communications from your care team through the platform.

Consent and withdrawal

  • You may withhold or withdraw this Consent at any time by emailing care@jivehealth.co with that instruction.
  • Withdrawal is effective upon receipt of your written notice. It does not affect any action taken by us or your Provider in reliance on this Consent before receipt, and does not affect any other provision of this Consent.
  • Otherwise, this Consent is considered renewed upon each new telehealth consultation with a Provider.

Electronic communication and privacy

The services necessarily involve electronic transmission of your personal and medical information. That information is handled as described in our Privacy Policy and, where applicable, its Consumer Health Data section. Information held by the Medical Group may constitute protected health information governed by HIPAA and the Medical Group’s own Notice of Privacy Practices. By using the services, you acknowledge receipt of that Notice.

We and the Medical Group will share your telehealth record with your other health care providers only with your consent and at your request. You may request that your telehealth record be sent to another provider by emailing care@jivehealth.co with your consent along with the provider’s name, address, and phone number. You also have access to your health and wellness information from your telehealth consultations in accordance with applicable law.

If a technical failure affecting the services results in a loss of your information or an interruption of your care, you agree to hold Jive Health harmless for any resulting loss of information or delay in care, in addition to any disclaimers in our Terms of Service. We do not and cannot guarantee the security or privacy of the services you use to receive communications, including your email service provider.

Financial considerations

  • You remain free to seek examination and treatment from any other healthcare provider at any time.
  • Prescriptions issued through the services are filled by partner pharmacies and shipped to you.
  • You are responsible for the cost of the services, including prescriptions, and agree not to submit claims for these services to any federal or state healthcare program (including Medicare or Medicaid) or seek reimbursement from such programs.
